CHECK LIST

Renewal of Registration.

 

 

The following details / documents have to be submitted by the MTO in connection with his application for renewal of MTO Registration :-

 

1.                   Application in Form-III duly completed in all respects and signed by the authorized signatory.

 

2.                   Annual turnover for the last year or average turnover during the last three years.

 

3.                   Registration number and date of registration.

 

4.                   Date of last renewal if any.

 

5.                   Statement showing the Multi-modal contracts entered into and the MTDs issued since registration/last renewal.

 

6.                   Separate CA Certificate within the CA Act, 1949 for the last 3 financial years.

 

7.                   Copy of the ‘valid liability insurance cover’ along with the application of renewal of registration.

 

8.                   Few copies of MTDs issued during the period of last renewal.

 

9.                   A specimen copy of MTD.

 

10.               An under-taking as per format enclosed.

 

11.               Original MTO Registration certificate.

 

12.               A demand draft of Rs.10,000/- in favour of DG(S), Mumbai towards renewal fees for 3 years.

__________________________ is a registered Multi-modal Transport Operator bearing No. ________________________ dated ____________ which is valid for the period from ___________ to ___________.  In the event of Renewal of Registration as MTO, we hereby undertake to issue Multi-modal Transport Documents only as prescribed by the D.G. Shipping under the provisions of Section 7 of the MMTG Act, 1993 and Rules thereunder and no any other document for all multi-modal transactions.  The MTD so issued will be under a valid liability insurance cover as required.
