CHECK LIST FOR APPLICANTS SEEKING REGISTRATION AS A MULTIMODAL TRANSPORT OPERATOR
1.
Application in Form-I
duly completed in all respect and singed by the authorized signatory.
2.
A copy of the Memorandum & Articles of Association, if
the Company is Private Ltd./Limited or proof of a Firm/Proprietary concern (Any
applicant who is not resident of India and is not engaged in the business of
Shipping shall not be granted registration unless he has established a place of
business in India).
3.
Names, addresses and other relevant details of the branches
in India and abroad.
4.
Statement indicating the assets/infrastructure available
like the building space, communication capabilities.
5.
Separate statement showing the staff details including their
qualifications and experience in the present business of the concern.
6.
Audited Balance Sheet for the last financial years with
Certificate from a Chartered Accountant in the case of a Company.
7.
Capital balance of the partners in the case of firm or
individual owner in the case of proprietary concern.
8.
Annual turnover for the last year or average turnover during
the last three years in case of a shipping or a freight forwarding concern
(Turnover means total freight amounts collected from shippers/consignors for
multi-modal transportation as defined in the Act).
9.
In case of a new shipping or freight forwarding concern,
highest turnover during the last six months.
10.
Copies of agency/representative agreements from at-least two
different foreign countries. The
Agreements should cover the following :-
(a)
Applicant concern should act as a ‘Principal’ and the
foreign counterpart will act as ‘Agent’.
(b)
Date of Signing the Agreement should be specifically
mentioned.
(c)
Validity of the above Agreement.
(d)
Confirmation of agreements by foreign Agents in addition to
the agreements.
11.
The applicant concern must submit proof copy of the ‘Valid
Insurance Policy’ at the time of grant of Registration Certificate.
12.
Bank draft for the applicable fee of Rs.10,000/- drawn in
favour of D.G. Shipping, Mumbai, drawn on any Nationalised Bank.
13.
An under-taking as per format enclosed for issue of MTDs on
getting MTO registration.

In the event of being granted the Registration as a Multi-modal Transport Operator, we hereby undertake to issue Multi-modal Transport Documents only as prescribed by the D.G. Shipping under the provisions of Section 7 of the MMTG Act, 1993 and Rules thereunder and no any other document for all multi-modal transportations. The MTD so issued will be under a valid liability insurance cover as required.
