Sub: Issue of fresh CDCs & Renewal, Duplicate and Replacement CDCs etc..

 Consequent to promulgation of M.S. Notice No. 18 of 2002 dated 5th September, 2002 prescribing forms - I and II for issue of CDCs to Cadets, TMEs, Ratings etc. through the Maritime Training Institutes, which conduct the pre-sea training courses approved by the Director General of Shipping, there is an urgent need to revise the existing CDC application form.

Accordingly, in pursuance of sub clause 2 of Rule 3 of Merchant Shipping (CDC) Rules, 2001, the Director General of Shipping is hereby pleased to prescribe Forms A & B for issue of fresh CDCs (other than candidates who have undergone DGS approved courses) and for renewal, duplicate / replacement CDCs, respectively. The Forms A & B are placed at Appendices A & B, respectively, and will come into force with immediate effect.
