Subject: Application Form for fresh Continuous Discharge Certificates
            (CDCs), renewal
, duplicate & replacement

         
 

  In pursuance of sub clause 2 of Rule 3 of Merchant Shipping (CDC Rules), 2001, CDC application Form A & Form B and its guidelines for the issuance of fresh CDCs (other than candidates who have undergone DGS approved pre sea training courses)  and for renewal, duplicate/ replacement CDCs, respectively were promulgated vide M.S. Notice 25 of 2002 by the Directorate.

       Consequent upon commencement of processing & the storage of data of the seafarers' in  electronic format, details such as INDOS No., STCW certificates are required to be included in the format.  In addition,  the contact details of the seafarers' are also necessary for creating the data base for efficient communication with the seafarers'.  Accordingly, the Director General of Shipping is pleased to revise the Form A & Form B along with its guidelines for the issuance of fresh CDCs  (other than candidates who have undergone DGS approved pre sea training courses)  and for renewal, duplicate/ replacement CDCs. The revised Form A & B and guidelines, appended with this notice shall come into force with immediate effect.
