Application For Duplicate copy
for loss of certificate of competency
The candidate will approach
the MMD form where the certificate was issued. He will fill up
FORM 16 (previously form H) completely. Alongwith the form he
will submit (i)a report from the police station under whose jurisdiction
the place where certificate is lost falls, (ii) an advertisement
of newspaper, (iii) latest xerox copy of the certificate available.
He shall pay the requisite fees for the certificate.
MMD will send the application
form(form16) alongwith all the documents to D.G. Shipping where
it will be checked with the duplicate of the original.
If everything is found proper
then the certificate will be prepared after three months. The
certificate will be marked as "first copy".
The certificate will then be
sent to MMD for issue.
