            Enclosed herewith is the questionnaire to be kept in the exam center.  This questionnaire is a Candidate's Feed Back Form which will assist us to analyse the drawbacks in existing exam system.  A separate box be kept so that the candidates can drop the form in the box.  We expect that the candidate
give their candid views on this subject.  They should be encouraged to do so. Wide publicity may be give to this form. 
The training institutes may also be informed about the same, and if possible, the shipping magazines may be requested to publish the form.  The forms may please be forwarded to
